What is a Service Plan?
- a meeting once a year
- discusses all of your services and supports for the next year, example – job coach, day program, cooking program, money management,
- should be based on what YOU want/need
How to get ready for your Service Plan?
- What are my goals? For example – work, friends, apartment, drive a car, go on a trip, different day program, move, etc.
- What is most important to me?
- Do I have any ideas about how to get to the goal?
- What am I good at?
- What can I do?
- What do I need help with to reach my goal?
- Remember, you have services and supports that can help you. Ask what can be in place to help you.
- Would it help to write things down to talk about?
- Can you talk to your RC about what you want to talk about before the meeting?
The Service Plan meeting:
- Bring up what you want to talk about. Bring a list.
- Have the team talk about your concerns and ideas completely.
- If you don’t understand, ask questions or say, “I don’t understand.”
- Goals – these should be what YOU want
- ISSPs – these are the programs day and residential are to work on with you. Do they help me reach my goals?
- Be responsible. Tell people what you can do to help yourself.
- Hold people responsible. Make sure it is in writing. Who is going to do what? When can they do this by?
What do I want to talk about at my meeting?
Make a list of things you want to talk about at your meeting prior to attending (ie. home, school, work, family, friends, activities, fun things)
