A service plan is a meeting once a year which discusses all of your services and supports for the next year. Your service plan should be based on what YOU want/need. 

Preparation for Your Service Plan
  • Define your goals: work, friends, housing, driving, vacations, different day program, relocating, etc.

  • Define what is MOST important to you

  • Ideas about obtaining your goal(s)

  • What are you able to do

  • What are your strengths & weaknesses

  • What help do you need to obtain your goals?

  • Ask how services and supports can be put in place to help you

  • Write everything down

  • Speak with your Resource Coordinator before your meeting

  • Invite people to the Service Plan meeting who are important to you; it's your meeting!

The Service Plan Meeting
  • Bring a list of what you want to talk about

  • Have the team talk about your concerns & ideas

  • If you don't understand - ask questions or simply say "I don't understand"

  • Make sure the goals are what YOU want

  • Be responsible. Tell people what you're capable of doing yourself

  • Hold others responsible - ensure it's in writing

  • Make sure to get contact information for those who have helped and those who could potentially help you with your goals
